COVID-19 Reporting, Contact Tracing, and Testing

The self-reporting form is to notify CSP’s Rapid Response Team of persons believed to be affected by COVID-19.

Information transmitted by this form will be used by the University’s Response Team to determine appropriate follow-up for addressing the needs of the campus community.

Contact Tracing / Cluster Process
Concordia University’s contact tracing team is composed of key officials on campus who represent key student and staff groups on campus from academic, student life, residence life, athletics, human resources and administration. If you have a case brought to your attention, this can be communicated to the committee through the COVID-19 Reporting Form or emailing the committee directly.

The committee is called to action when a COVID-19 lab-confirmed positive case is submitted and communicated through the Minnesota Department of Health (MDH) or submitted via the COVID-19 Reporting Form.

The committee’s action plan includes steps that involve pulling a report with specific student data that assists with contact tracing to determine close contacts, the team reporting to MDH and notifying individuals if they were reported as close contacts to the MDH and the MDH notifying individuals who may be at higher risk of infection.

The group also works through clusters, where 20-to-25 percent of a group (class, athletics workout group, any other campus group) have a COVID-19 positive lab-confirmed case simultaneously. In that scenario, the Contact Tracing Team works with the necessary staff/faculty in charge of that group to determine if activities should be paused or moved online. In larger settings, the Contact Tracing Team would also work with the MDH to determine potential pause or changes to in-person activity.
